The immune reconstruction inflammatory syndrome (IRIS) or immune reconstitution inflammatory is an inflammatory phenomenon resulting from the rapid restoration of an excessive and uncontrolled immune inflammatory response. At first, this syndrome was described in HIV patients, as a response to the onset of retroviral therapy, but subsequently cases have been described in patients with tuberculosis, where this was initially called paradoxical reaction of the tuberculosis and in cases of other pathogenic microorganisms. The symptoms are variable and most follow a mild course. There is no diagnostic gold for IRIS. In general, the diagnosis is made by exclusion. Most of the cases are mild and undergo spontaneous resolution with the passing of the days. In the severe cases, treatment may be required with corticosteroids or anti-inflammatory drugs in order to stop the inflammatory phenomena.
